Criminal Record Check
1. Go to your local police station (or the place that the local police have designated for criminal record checks). It is recommended that this be done approximately three months prior to the first day of your practicum.
2. Fill out a Criminal History Consent for Disclosure Form that authorizes the police to search their records and provide information about whether or not you have a criminal record. The completed form will indicate if you have a criminal record and/or if local police records are present. This criminal record check will cost approximately $50.00 and most police stations accept CASH ONLY. You are responsible for this fee and you will need to show two pieces of identification (one piece must be picture I.D.).
3. Please have the police mail/fax the document, or you can mail or hand deliver the original document, to:

4. The Form will be returned to you once it has been reviewed and recorded. If the Form indicates that there may be a record of information held by the justice system, the Practicum Placement Coordinator will direct you to contact the College Safety Officer.
The College Safety Officer will discuss the nature of the information with you and attempt to validate the information. This information is held in strict confidence and will not be shared with any member of College Faculty or Support Staff.
Depending upon the nature of the criminal record, and its relationship to the program in which you are enrolled, a student who has a criminal record may find it difficult or even impossible to be accepted in practicum placements or to find related employment. The inability to be placed in a practicum will result in failure from the program.
