| After you have submitted the form, please fax or mail us the following documents in order to complete your application. |
- Application Fee (non-refundable) of C$150 (C$200 for International Students) sent via Bank Draft or Money Order, made payable to Gateway College. For alternate forms of payment, please call or write to Admissions Office at 604-738-0285.

- Secondary School Diploma or Post-Secondary School Diploma
- Official Transcripts
- Photocopy of Birth Certificate or Passport
- Photocopy of Picture ID (Passport or Driver's License)
- Two Passport Sized Photographs
- A proof of language proficiency
- TB Test Result (only for Health Care Programs)
- Criminal Record Check Report (only for Health Care Programs)
